Volunteer Application

By signing this application, I agree to give my best effort and fulfill the written work detail assignments I have been assigned and oral assignments given to me by the Volunteer Coordinator and other festival officials from time to time. I also agree to hold harmless Garlic, Inc, its officers, volunteers, successors and assigns for liability due to accident, negligence, gross negligence, loss, theft and all other claims and I release them from all damages, known and unknown. If I do have a claim, I agree to submit it in writing within 30 days from the event, otherwise, I release those involved and the above mentioned. I affirm that I am 18 years old or older. I agree to attend a mandatory job orientation meeting (about 1 hour) prior to the event (time and date tba). I also agree to timely notify the volunteer coordinator if my availability changes for the position indicated on this application. I understand that Garlic, Inc. will attempt to give me the shift and duty I requested above, but they may not be able to do so. I also understand they will be relying on my timely performance. There will be an orientation meeting on or about August 27th – exact times and places to be announced. I have read and understand the above statement and agree to it.
